About the role

As a Disability Day Service Practitioner you will provide high quality community based and individualised services that support and promote community inclusion, personal growth and living skills for people with a disability.

Duties

  • Provide support to clients to achieve their personal goals through group and individual based supports;
  • Meet the expectations and needs of program members, establish, and maintain effective relationships;
  • Develop and review programs and activities for participants in alignment with Person Centred Plans;
  • Provide support to program members to participate as far as possible in community activities where appropriate;
  • Be proactive in promoting community inclusion through following up on community based opportunities for program members based on their particular interests and needs.
We have part-time positions available in Geelong.

Skills and experience

To be successful in this role, it is a requirement that you have:
  • Certificate III or IV in Community Services (Disability) or equivalent;
  • First Aid Level II;
  • NDIS Worker Screening Check clearance;
  • Intermediate computer skills – Word, Excel, Internet and Outlook;
  • Knowledge of, and ability to implement the Disability Services Act and Standards;
  • Current Working with Children Check or willing to obtain;
  • Current Drivers License.
